2018 Clerk's Annual Questionnaire (CAQ)

Welcome to the 2018 Clerk's Annual Questionnaire (CAQ)!


Here is some useful information about this survey:

1 Gather your information before entering your responses into the online survey form. You may choose to have different people from your congregation respond to different parts of the survey. Once you have the information, it should take about 30 minutes to complete the survey.

2 You can use the "Back" button within the survey to return to previous pages. The data will not be lost. Your information will be saved as you move from question to question. Do not, however, click on the back arrow in your Internet browser; in that case, your responses may be lost.

3 Once you have reached the end of the CAQ, you will have an opportunity to review your responses and make changes before you submit the CAQ.

4 Once you submit the CAQ, an email will be sent to you with a PDF file of your responses. This is the copy of your responses that you are able to save in your files and/or print. The sender will be listed as Deb Worland.


The deadline for submitting your responses to the CAQ is Thursday, November 15, 2018.

6 If you have any questions concerning the CAQ, you may send an email to 
CAQ@pcusa.org  or call (800) 728-7228, ext. 2040.

7 To print a copy of the CAQ before starting the survey, click the following link or copy and paste it into your browser. Your link to the CAQ will remain active or open while you view and/or print the PDF. After viewing the PDF, return here to complete CAQ online. This link allows you to print the PDF; you cannot enter your responses on the PDF. To continue without viewing the PDF, scroll to the bottom of the page then click "NEXT."
